accredited Home Health Agency serving Sarasota and Charlotte counties. We
are locally owned and operated and, as such, can relate specifically to the
concerns and problems in our community. Our organization is committed to

Mission Statement
Doctor's Choice Home Care, Inc. is committed to providing a level of quality
care that encourages the safe and cost-effective delivery of home health
services, maximizing patient autonomy, and coordination of community and Agency
resources, to decrease illness burden and unnecessary duplication of services,
thereby promoting positive patient outcomes. Experienced, knowledgeable staff formulate an individualized plan of care
which embodies skilled services, personal care, psychological support and
education within guidelines established by a physician's orders. Patients
and families are actively encouraged to participate in this multi-disciplinary
approach to ensure that short term and long term goals for the patient are
attainable.
